This course is aimed at trainees and young surgeons interested in patient selection, therapy selection, and proper application of surgical therapy.

The treatment of common proctologic problems such as hemorrhoids and anal fistulas can seem simple and straightforward at first glance. Interestingly, the more trainees are exposed to it, the more problems often arise. Anatomy can be complex, patients can be demanding and results of therapy can be disappointing.

Course Content

An overview of different methods will be given by international experts. Also, the possibility to gain some hands-on experience on the world-famous and unique 'Isbert model' will be offered.

Learning Objectives

To give attendees a better understanding of relevant anatomy and pathology where applicable. A working knowledge of available and basic techniques for the treatment of perianal stulas and haemorrhoids and a better understanding of the selection process for appropriate therapy modality.

Part 1: Haemorrhoids

9:05

Pathophysiology and general considerations

Baljit Singh (UK)

Why do our patients get haemorrhoids? What evidence is available? What is bothering them most? What is expected from us in the treatment of haemorrhoids?

9:30

Selection of appropriate technique for your patient

Stanley Goldberg (USA)

What different mainstay techniques are currently available? What techniques can be offered to our patients on an outpatient basis? What operative techniques are available and what technique should be applied for the individual patient? What pittfalls should we worry about?

10:15 Coffee and Tea Break
10:30

Modern insights and techniques in operative treatment for haemorrhoids

Steve Brown (UK)

Recently, several studies concerning the treatment of haemorrhoids were published (Ethos , Hubble). What do these studies mean for daily practice? Should all modern techniques such as HAL and PPH be abandonded?

11:15

Hands-on practice of different techniques for haemorrhoid treatment

Christoph Isbert (Germany)

12:15 Lunch

 Part 2: Perianal Fistula

13:00

Pathophysiology and general considerations

Christoph Isbert (Germany)

Why do our patients get fistulas? What evidence is available? What is bothering them most? What is expected from us in the treatment of Fistulas? Are there new insights in pathophysiology? Why are new techniques not working?

13:30

14:15

Modern insights and techniques in operative treatment for perianal fistula

Mia Kim (Germany)

Many new methods are available, but it is rare for new techniques to last longer than 5 years. Considering pathophysiology, what techniques are available, why should they work and what should we be offering our patients?
